Most people know that the game was named 2 different things. In the USA it was named "Sonic 3D Blast", while in Europe it was named "Sonic 3D Flickies' Island". But what a lot of people don't know is, the ROM for the 2 carts ar EXACTLY the same. They just react to the hardware, and choose the correct name. So if you had a Genesis from the US, and a Megadrive from the UK, and tried the cart in both, it would react different to each. This can be reproduced in a emulator. Select the setting "USA hardware" and play it. You get Blast. , now select "Europe hardware". You get Flickies' Island. Also, if you ever get your hands on the PC version, it also reacts to where you live the same way.
